EmpowerMe therapists perform on-site safety evaluations and create balance and fall reduction programs.   • After a fall and hip surgery, G. was fearful of walking and relied heavily on her husband and a wheelchair, which limited her activity and put her at risk of further decline. With just 12 on-site physical therapy sessions, support from her family, and a personalized exercise plan, she regained her confidence and mobility, now walking independently with her walker, attending meals, and enjoying outings with her family.

  • M. began on-site occupational therapy to improve her hand strength and fine motor skills, which were affecting her ability to open jars and packages, and also addressed decreased mobility due to back pain. As a result of her hard work, she can now walk longer distances, get to appointments more easily, and independently manage daily tasks in her apartment.

  • C. began therapy due to memory lapses that disrupted her daily routine. With personalized memory aids, structured routines, and cognitive activities like puzzles and reading, she’s regained confidence and independence while continuing to enjoy the things she loves.

  • Nan moved in with her husband, who has since moved to memory care. Nan has chronic neck pain from not sleeping while she took care of her husband who stays up most nights and sleeps during the day. Once he moved, Nan approached us right away wanting treatment to take care of her neck pain once and for all. Her neck pain is slowly going away and she has a new lease on life, attending every event she wants to now.

  • A resident who is not on caseload, but uses our Wellness Center daily, said she loves all the extra equipment we can use in the gym and is noticing that many more residents are utilizing the Wellness Center since the EmpowerMe team arrived and is able to show us how to use the equipment.

  • The therapists are all caring and very flexible when my mother’s schedule suddenly changes. They even try to steer clear of the time Mom likes to watch “The Price is Right”. They’re easy to talk to and discuss what they’re doing to help Mom regain strength and flexibility. I’m so thankful they told me about the 360 program and are proactive to get Mom into this extra care program. I really appreciate them and all they do.

  • EmpowerMe collaborated with our community to provide activities support in the absence of an activities director using the wellness program, and regularly assisting with kick ball, balloon ball, and happy hour for community wellness.

  • Dorothy was recently discharged from therapy services and continued with the 360 program. She usually does not spend much time outside of her apartment. She is visually impaired and needs reminders for upcoming and ongoing activities. With assist of the 360 program, we have been able to allocate preferred activities within Primorse with use of an enlarged calendar and daily reminders on participation. Sometimes we notice she is now even out of her apartment more frequently, whether she is getting an extra walk in or going to activity!
